Letter of Appointment – General

For MBIB to act as your General Insurance Broker you need to formally appoint us by completing the letter of appointment below.

Please note:

  • LOA’s must be signed by a director, financial controller, or owner of the buinsess

  • The LOA must be printed on your business letterhead (if you do not have letterhead, please include your full name, business name and address at the top of the LOA)

  • LOA’s must be dated as they have a validity period

Letter of Appointment – Home Building Compensation Fund (HBCF) (NSW)

For MBIB to act as your Home Building Compensation Fund (HBCF) broker you need to formally appoint us by completing the letter of appointment below.

Please note:

  • LOA’s must be signed by a director, financial controller, or owner of the business
  • LOA’s must be dated as they have a validity period
Letter of Appointment – Building Indemnity Insurance (SA)
For MBIB to act as your Building Indemnity Insurance broker you need to formally appoint us by completing the letter of appointment below.

Letter of Appointment – Home Indemnity Insurance (WA)

For MBIB to act as your Home Indemnity Insurance broker you need to formally appoint us by completing the letter of appointment below.

Please note:

  • LOA’s must be signed by a director, financial controller, or owner of the business,

  • LOA’s must be dated as they have a validity period.

Letter of Appointment – Residential Builders’ Warranty Insurance (ACT)

For MBIB to act as your Residential Builders’ Warranty Insurance broker you need to formally appoint us by completing the letter of appointment below.

Please note:

  • LOA’s must be signed by a director, financial controller, or owner of the business,

  • LOA’s must be dated as they have a validity period.

Domestic Building Insurance (DBI) Distributor Nomination Form VMIA (VIC)

For MBIB to act as your DBI distributor you need to formally nominate us by completing the Distributor Nomination form below.

Please note:

  • You must nominate a DBI distributor to access or set up a BuildVic account,
  • If you are a new builder, you must undergo an underwriting assessment which will determine your eligibility to purchase Domestic Building Insurance.
Residential Builders’ Warranty Insurance Eligibility Application Form (ACT)

Builders require a Letter of Eligibility from an approved Home Warranty Insurer in order to qualify for and/or renew their Building License in most Australian states and territories.

Upon receipt of this completed application form our underwriters will undertake an assessment of your financial position and technical qualifications/experience as part of the approval process.

Approved applicants will then be issued with an Letter of Eligibility to enable them to comply with relevant law relating to the state/territory in which they operate.

Home Building Compensation Fund (HBCF) Eligibility Application and Profile Change Form (NSW)

If you are a new builder, you must undergo an underwriting assessment which will determine your eligibility to purchase Home Owner Warranty in New South Wales.

Your Certificate of Eligibility determines the maximum value and number of construction projects you can have open at any given time with icare HBCF.

It also certifies the maximum contract price for each type of construction work you’re eligible to do under your approved eligibility.

You should apply for your Certificate of Insurance before commencing work on any project and before taking any money, including a deposit under the building contract.

To obtain a certificate of DBI insurance, all licensed builders must first have a Certificate of Eligibility.
